Thank you for considering Central Vet for your animal’s care!

We are currently booking more than six weeks in advance for new clients and their animals. At Central Vet, we are grateful for our loyal client base and work hard to ensure we are available when our patients need us most despite this high demand. To register as a client, you must complete and submit* the following digital forms so that we can create your account (alternatively, these forms may be sent in PDF/Word format or printed upon request and then returned either via email or person):

Client Information Form

This allows us to get to know you and your pet(s) and give you information on the services and communication you’ll receive from us.

This allows us to get all of the important medical information about your pet.

Authorization To Release Medical Records

This allows us to request your animal’s previous veterinary records on your behalf in preparation for your appointment.

Once we receive your completed forms, you and your animal’s information will be placed into a queue, and our team will contact you when an appointment becomes available. Please disclose within the ‘Patient Information Form’ your current concerns regarding your animal’s health so that we can gauge the urgency of the appointment, as this will allow us to select an appropriate appointment offering when the time comes.

We are located just outside of Winnipeg on the South Perimeter Highway, 3 miles East of Oak Bluff. Look for our sign on the highway! To get directions and to view our hours click here.
